Job Description
Registered Nurse (RN) in Medical/Surgical unit at Ascension Via Christi St. Francis hospital in Wichita, KS.
Required Credentials & Onboarding Requirements
Licenses & Certifications
- Nursys Verification: Must be completed within 30 days prior to start. Complete FACIS Questionnaire if any disciplinary actions.
- Current Nursing License: Primary source verification within 30 days prior to start. FACIS Questionnaire required if disciplinary actions exist.
- BLS (AHA): Required prior to onboarding.
- MOAB Moderate Risk De-Escalation Training: Required prior to onboarding.
- NIH Certification: Group A updated yearly; Groups B-F updated every 2 years.
- Ascension EKG Exam: Must pass with at least 80%, max two attempts. Updated version required as of Dec 1, 2023.
- Ascension Cerner Test-Out: Must complete test and training checklist with signed attestation.
- Ascension 12 Panel Drug Screen: Within 30 days prior to start, updated annually. Includes testing for 12 specified drugs with MRO review if applicable.
- Ascension Skills Checklist: Clinical skills checklist completed within one year of start, updated annually.
- Ascension Education Verification: Verification of clinical education within 90 days of start; must be rerun if break in service > 90 days.
- Ascension Criminal Background Check: Includes SSN trace, employment verification, OIG search, national sex offender check, updated annually or if break in service > 90 days.
- Ascension EPLS Check: Run within 90 days of start; rerun if break in service > 90 days.
Health & Immunization Requirements
- MMR: Documented proof of two vaccines or positive IGG titer. No declinations accepted. If negative titer, two doses four weeks apart required.
- Varicella: Proof of two vaccines, immunity, or verified history. No declinations accepted. If negative, two doses four weeks apart required.
- Tdap: Valid within 10 years. No declinations accepted.
- Hepatitis B: Documentation of vaccination series completion or signed declination.
- Influenza Vaccine: Optional but recommended during flu season (Oct 1 - Mar 31). Declinations accepted with mask requirement.
- Positive PPD: Negative chest X-ray within one year and annual TB questionnaire required.
- 2 Step PPD or IGRA: 1st PPD within 1 year, 2nd within 30 days of start or negative IGRA within 90 days. Annual TB questionnaire required.
- Fit Test: Required for all travelers, preferably before start; onsite completion possible (except PAPR). Annual update required.
- Health Statement: Statement of good health or physical exam prior to first assignment, updated annually.
Professional & Personal Documentation
- Resume/Profile: Required for submission.
- Two Professional References: Required prior to first assignment.
- Driver’s License/State ID: Must be current, match address on file, and show front/back.
- Ascension Badge Photo: Forward-facing headshot with white or blue background, no alterations.
- Ascension Onboarding Requirements Education Attestation: Completion of 6-hour pre-hire module with attestation.
- Ascension Nursing Contract Worker Onboarding: Completed onsite.
- Ascension Nova StatStrip: Pre-hire modules required for non-allied positions.
- Ascension Confirmation of Assignment: Provided upon initial assignment and updates with extensions.
- Ascension Employment Verification: Verification of employment for past 7 years, updated annually or if break in service > 90 days.
Optional / Additional Information
- Ascension Unit Specific Checklist: Optional for onboarding; uploaded by unit leader within a week.
- Influenza Vaccine Exemption/Declination: Accepted with mask requirement during flu season.
- Positive PPD - Negative Chest X-Ray and Annual TB Questionnaire: Optional for submission but required if applicable.
- Hiring Manager Evaluation: Post-assignment evaluation (optional).
