Operating Room Registered Nurse

Location: Willoughby, Ohio
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Job Details

• Full-time 4x10 hour shifts, Monday to Friday, 7:00 AM – 5:30 PM, with varied shift days.
• Call requirement: minimum 1 weekday per week and 1-2 weekend days per month (Saturday or Sunday).
• Provide RN care to surgical patients at a Level III trauma center referral hub.

Required Qualifications

Licensure

• Valid RN license in Ohio.

Certifications

• BLS issued by American Heart Association.
• ACLS issued by American Heart Association.

Education

• Associate of Science in Nursing / Associate Degree in Nursing.

Experience

• OR - Operating Room (1 year of experience).
• OR - Operating Room experience within the last year.

Additional Requirements

• Driver's License.

Health and Immunization Requirements

• COVID-19 vaccine: Preferred but not required; medical or religious exemptions accepted with approval.
• MMR: Positive titers within last 15 years required; booster protocol applies if low titer.
• Varicella: Positive titers within last 15 years required; booster protocol applies if low titer.
• Tdap: Vaccination within last 10 years required; no declinations accepted.
• Hepatitis B: Proof of full series or partial series with signed declination accepted; positive titers within 15 years accepted.
• Influenza vaccine: Required October through March; declination requires mask-wearing and completion of specific forms.
• Tuberculosis testing: 2-step PPD or IGRA within last 6 months required annually; if positive, clear chest X-ray and current TB questionnaire required.
• Fit test for N95 respirators: Completed within 1 year prior to start and updated annually.

Background and Compliance

• Criminal background check within 6 months of start, including social security trace, county searches (7 years), national sex offender registry.
• Ohio fingerprinting and FBI background checks required based on residency duration.
• OIG exclusion check within 6 months prior to start.
• SAM/EPLS/GSA/OFAC screening within 6 months prior to start.
• Drug screen within 60 days prior to start; 10-panel urine drug screen required (instant screens not accepted).
• Employment verification for last 5 years through third-party.
• Education verification through third-party; international education requires equivalency documentation.
• I-9 attestation confirming eligibility to work in the U.S.
• Submission packet required: resume, 2 professional references (within last 2 years), job application, and employment history form.

Competency and Skills

• Skills checklist specific to clinical area completed within 1 year prior to start (one-time requirement).
• Competency exam for relevant department with minimum 80% score, completed within 1 year prior to start.
• Core competencies (HIPAA, Joint Commission NPSG, OSHA topics) with minimum 80% score, completed within 1 year prior to start.
• Completion of required safety and compliance training modules prior to clearance; annual renewal required.

• Physical exam within 1 year prior to start confirming ability to work without restrictions.
• Non-expired photo ID (driver’s license or state ID).
• Badge photo (headshot, shoulders up, plain background) to be uploaded as JPEG.

Shift

• 7:00 AM – 5:30 PM (varied days within Monday-Friday schedule).
