• Location: San Ramon, CA (serving areas including Santa Clara, Sunnyvale, Cupertino, Milpitas, Mountain View, Campbell, Los Altos, Saratoga, Los Gatos, San Jose)
• Employment Type: Travel/Contract
• Contract Length: 13 weeks
• Shift: Day shift, 8 hours per day (8:00 AM - 4:30 PM), 5 days a week
• Hours: 40 hours per week (guaranteed 36 hours weekly after ramp-up)
• Start Date: ASAP (compliance process takes about 2 weeks)
• Patient Load: 5-6 patients per day, 1:1 ratio, with a standard of 27 productivity points daily
• Weekend Work: Optional weekend patient visits to meet productivity goals
• On-Call: Not required
• Locals Accepted: Yes

Required Qualifications

• Valid Occupational Therapist license in California
• Minimum 1 year of Occupational Therapist experience within the last year
• Experience in a Home Health setting
• Current Basic Life Support (BLS) certification

Documentation & Compliance Requirements

• Background Checks (7-year history, nationwide, sex offender registry)
• Annual EPLS/GSA/SAM and HHS/OIG checks
• Certifications: BLS required; ACLS and PALS preferred but not mandatory
• State License verification (primary source online)
• Medication/Pharmacology Competency Exam as applicable
• Personal or Auto Insurance (current and valid throughout assignment)
• Auto Registration (current and valid throughout assignment)
• Physical Exam within 1 year, cleared to work without restrictions
• Immunizations and Titers:

  • Tdap within 10 years or annual declination
  • Negative TB test or chest X-ray within 1 year
  • Measles, Mumps, Rubella: 2 vaccines or positive titers within required timeframes
  • Hepatitis B: Titer within 5 years or vaccine series or declination
  • Varicella: 2 vaccines or positive titer within required timeframes
• Drug Screening: 10-panel drug screen within 1 year
• Flu Vaccine or Declination (required year-round)
• COVID-19 Vaccine documentation or declination form (religious/medical exemptions accepted)
• References: 2 professional references for travel/allied/per diem acute care roles (within 3 years)
• Work History Verification: 1 year related employment verification required
