Job Details
- Specialty: Therapy/Rehabilitation (Occupational Therapy)
- Location: San Ramon, CA (serving areas including Santa Clara, Sunnyvale, Cupertino, Milpitas, Mountain View, Campbell, Los Altos, Saratoga, Los Gatos, San Jose)
- Employment Type: Travel/Contract
- Contract Length: 13 weeks
- Shift: Day shift, 8 hours per day (8:00 AM - 4:30 PM), 5 days a week
- Hours: 40 hours per week (guaranteed 36 hours weekly after ramp-up)
- Start Date: ASAP (compliance process takes about 2 weeks)
- Patient Load: 5-6 patients per day, 1:1 ratio, with a standard of 27 productivity points daily
- Weekend Work: Optional weekend patient visits to meet productivity goals
- On-Call: Not required
- Locals Accepted: Yes
Required Qualifications
- Valid Occupational Therapist license in California
- Minimum 1 year of Occupational Therapist experience within the last year
- Experience in a Home Health setting
- Current Basic Life Support (BLS) certification
Documentation & Compliance Requirements

- Emergency Contact Form
- I9 Attestation
- Background Checks (7-year history, nationwide, sex offender registry)
- Annual EPLS/GSA/SAM and HHS/OIG checks
- Certifications: BLS required; ACLS and PALS preferred but not mandatory
- State License verification (primary source online)
- Medication/Pharmacology Competency Exam as applicable
- Personal or Auto Insurance (current and valid throughout assignment)
- Auto Registration (current and valid throughout assignment)
- Physical Exam within 1 year, cleared to work without restrictions
- Immunizations and Titers:
- Tdap within 10 years or annual declination
- Negative TB test or chest X-ray within 1 year
- Measles, Mumps, Rubella: 2 vaccines or positive titers within required timeframes
- Hepatitis B: Titer within 5 years or vaccine series or declination
- Varicella: 2 vaccines or positive titer within required timeframes
- Drug Screening: 10-panel drug screen within 1 year
- Flu Vaccine or Declination (required year-round)
- COVID-19 Vaccine documentation or declination form (religious/medical exemptions accepted)
- References: 2 professional references for travel/allied/per diem acute care roles (within 3 years)
- Work History Verification: 1 year related employment verification required
- Annual Skills Checklist
Additional Information
- Documentation tools will be provided and must be returned
- The system uses scheduling to minimize drive and windshield time
