Job Opportunity: Licensed Masters of Social Worker (LMSW)

Location

Portland, Oregon (East Portland and surrounding areas)

Employment Type

Travel/Contract

Contract Length

13 weeks

Work Schedule

Shift: Day shift, 8 hours per day (8:00 AM – 4:30 PM)
5 days per week (40 hours total)
No weekend requirement
No on-call duties
No floating between units
Facility cannot accommodate 4x10 or 3x12 shifts
Facility may call off up to one shift every two weeks

Job Description

Home Health Social Services role
Patient ratio: 1:1, with up to 6 patient visits per day expected
Productivity standard: 30 points per day
Charting system: EPIC Remote Client (laptop) and Rover (phone app) with Oasis documentation
Attire: Business casual preferred; scrubs allowed but no specific color required; no jeans
Parking: Free
Mileage reimbursement: IRS standard

Required Qualifications

Valid LMSW - Licensed Master Social Worker license in Oregon
Minimum 1 year of LMSW experience within the last year
Experience in a Home Health setting
Experience with OASIS documentation
Basic Life Support (BLS) certification
