Job Description

• Start date is subject to change based on nursing education availability.
• Local rate applies within 75 miles of the facility.
• Shifts: Monday to Friday, no weekends.

Requirements

• NY State Nursing License in hand.
• Minimum 2 years of infusion experience.
• Oncology experience preferred.
• ONS/ONCC Chemo certification preferred.

Certifications (may be required depending on unit)

• BLS (Basic Life Support) – Required for all RN positions.
• ACLS (Advanced Cardiac Life Support) – Required for ER RN positions.
• PALS (Pediatric Advanced Life Support) – Required for ER.
• NRP (Neonatal Resuscitation Procedure) – Required for Labor & Delivery, NICU, Mother/Baby.
• TNCC (Trauma Nursing Core Course) – May be required.
• EFM (Electronic Fetal Monitoring) – Required for Labor & Delivery, Mother/Baby.
• NIH Stroke Certification – May be required.
• State License Verification – Required within 30 days of start; license issues will result in cancellation.

Competencies

• Medication Test/Pharmacology Competency Exam: Minimum score 80%.
• Dysrhythmia/EKG Competency Exam: Minimum score 80% (required for EEG Tech, may be required for other units).

Required Documents & Checks

• Driver’s license or state ID with clear picture and address.
• 7-year background check including:

  • Social Security Trace
  • County search
  • National Sex Offender Registry
  • National Criminal Background
  • FACIS Level 3
  • OFAC
  • OMIG (NY Medicaid exclusions)
  • OIG and SAM
• EPLS/GSA/SAM and HHS/OIG checks within 30 days prior to start.
• Education verification: Diploma or transcript acceptable.
• New York State Medicaid Exclusion List check updated annually.
• 2 clinical references required (supervisor level or above, Charge RNs acceptable).
• Employment application covering 7 years with explanations for gaps; must include at least 1 year experience in specialty or comparable/higher acuity.
• Skills checklist current within the last year and applicable to position.

Health & Immunization Requirements

• Facility-specific physical exam form completed by a physician within 30 days of start (must include specific sections and signatures).
• Fit test for N95 mask (Halyard Fluidshield or 3M 1860) within 12 months of start.
• Negative TB test (QGold, TSpot, or PPD) within 1 year; if positive, clear chest x-ray within 12 months and questionnaire.
• Immunizations or positive IgG titers required for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Varicella
• Hepatitis B antibody and antigen results required (proof of 3-series alone not sufficient).
• Standard 9-panel drug screen within 30 days prior to start; rapid drug tests not accepted.
• Seasonal flu vaccination required October 1 - April 30 (declinations allowed for religious or medical reasons).
• COVID vaccination preferred (initial series).

Additional Notes

• Background hits and other issues will be reviewed on a case-by-case basis.
• Physical exam and drug testing must be initiated within 24-48 hours of offer acceptance and completed within 30 days prior to start.
• All certifications and health documents must be current and updated annually as required.
