Job Overview

Role Summary
- Provide professional nursing services to clients of all ages in a home health and hospice setting
- Responsibilities include admission, evaluation, and care plan development
- Work as part of a coordinated homecare team
Required Qualifications
- Valid RN license in Vermont
- Associate of Science in Nursing / Associate Degree in Nursing
- BLS
- Home Health (1 year of experience)
- Home Health experience within the last year
- Hospice experience
