Licensed Practical Nurse (LPN) – Medical Surgical (Travel/Contract)

Location: Rochester, New York
Shift: Day shift, 4 days per week, 10:00 AM – 7:00 PM (4x10 hours)
Contract Length: 13 weeks
Weekend Requirement: Every other weekend
Holiday Requirement: Available to work every other holiday, if needed


Job Overview

  • Work in a discharge lounge setting with medical-surgical patients including COPD exacerbation, infections, failure to thrive, and social issues.
  • Patient ratio approximately 1:5-6 during day shifts.
  • Floating within skillset may be required.
  • Uniform colors: navy blue and white combinations.
  • Charting system: EPIC experience required.

Required Qualifications

  • Valid LPN / LVN license in New York (NY is not a compact state).
  • Minimum of 2 years LPN / LVN experience, including experience within the last year.
  • Basic Life Support (BLS) certification issued by the American Heart Association.
  • Medical-surgical nursing experience.
  • Experience with EPIC charting system.
  • Valid Driver's License.

Clinical Skills & Responsibilities

  • Vital signs, pain assessment, intake/output monitoring, isolation precautions, physical restraints, specimen labeling.
  • Oxygen administration via nasal cannula, feeding tube management, telemetry monitoring, 12-lead EKG.
  • Medication administration: oral, IM, SQ, NG/G-tube, ear and eye drops.
  • IV therapy with pumps, blood glucose monitoring.
  • Basic wound care including simple dressings and JP/Hemovac drains.
  • Bathing, mouth care, Foley catheter insertion (male/female), clean catch urine collection.
  • Communicate status changes to RN/MD.
  • Experience in medical-surgical and telemetry care settings.

Scheduling Details

  • Must work Thursday and Friday; other days negotiable.
  • Submit time-off and special scheduling requests upon application.
  • Facility may cancel up to three shifts during the 13-week contract without penalty.

Required Documentation & Compliance

  • Government-issued photo ID required.
  • 7-year criminal background check within 3 months of start, including SSN trace, national criminal, sex offender, and education verification from primary source.
  • EPLS/GSA/SAM and HHS/OIG checks required within 3 months and updated monthly.
  • Foreign education equivalency evaluation if applicable.
  • Education verification required (not applicable for CNA).
  • Primary source verification of state license within 30 days of start.
  • NURSYS verification for LPN license within 30 days of start.
  • Medication/pharmacology competency exam as required.
  • Color blindness testing required once.
  • Orientation safety manual acknowledgement required.
  • Facility-specific documents including photo for badge and parking pass form.
  • Physical exam within 1 year of start, updated annually.
  • Fit mask test within 1 year, updated annually (specific mask types accepted).
  • Negative TB test or chest X-ray within 1 year; TB questionnaire accepted in lieu of annual renewal.
  • Immunizations or positive titers required for: Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella.
  • Drug screening within 30 days of start.
  • Seasonal flu vaccination or signed declination required during flu season.
  • Snellen eye exam within 1 year of start.
  • COVID vaccination copy preferred but not required.

Additional Requirements

  • Employment application and skills checklist required annually.
  • Work history verification required (minimum two past employments).
  • References required for travel agency compliance (2 references within 3 years).
  • Fingerprinting and additional background checks may be required for certain units (mental health, behavioral health, home health aide, long-term care).
