- Day Shift: 6:30 AM – 3:00 PM (5 days/week)
- Evening Shift: 2:30 PM – 11:00 PM (5 days/week)
- Hours per Week: 40 hours
- Setting: Inpatient/Acute Care
Required Qualifications
- Valid Clinical Pharmacist license in Wisconsin
- Minimum 1 year of clinical pharmacist experience within the last year
- Doctor of Pharmacy (PharmD) degree
- Experience with Epic electronic health record system
Important Requirements & Conditions
- Travelers must shave to ensure proper mask seal for fit testing; use of personal CAPRs or PAPRs is not allowed, and these are not provided onsite
- Scrubs provided
- Fit testing will be completed onsite during the first week of orientation; failure to comply may result in assignment cancellation
- OSHA Respirator Clearance required prior to start (covered for Aya candidates)
- Negative drug screen within 12 months of start required
- Immunization and health documentation required, including:
- Flu vaccine or approved medical/religious exemption (must be renewed annually)
- TB test or chest x-ray within 12 months
- COVID vaccine series preferred
- Rubeola, Mumps, Rubella, Varicella titers or vaccination proof (Aya standard)
- Hepatitis B titer, series, or declination (required for new candidates)
- Background check within 12 months required, including SSN trace, 7-year county and nationwide search, sex offender search
- Completion of facility-specific orientation modules and documents prior to start and annually thereafter
- Completion of post-start orientation checklists during first week on assignment
- Two professional references required (Aya standard)
- Skills checklist required
Additional Notes
- Candidates must complete all required compliance modules assigned based on unit/position
- Certain specialized trainings (e.g., AbioMed Impella Online Training) required for ICU/CCU and Cath Lab candidates
- Various state-specific background and caregiver searches apply if working in Wisconsin or Missouri
- Photo for badge required except for certain Wisconsin facilities
- Candidates must sign confidentiality, security agreements, and standards of ethical conduct prior to start
