Registered Nurse (RN) — Home Health

Location

• Littleton, New Hampshire
• South Territory: Littleton/Plymouth areas

Contract

• 13 weeks
• Temp assignment
• Day shift
• 8 hours per day

Role Summary

• Provide home health and hospice nursing care in patients’ homes
• Create individualized plans of care
• Work as part of a multidisciplinary team with a high degree of autonomy

Required Qualifications

• Valid RN license in New Hampshire
• Associate of Science in Nursing / Associate Degree in Nursing
• Home Health (1.5 years of experience)
• Home Health experience within the last year
• Experience in a Home Health setting
• BLS issued by American Heart Association
• Driver's License
• Hospice experience

Preferred Qualifications

• Previous hospice experience
• 2–5 years of med-surg experience
• Hospice skills checklist

Submission Requirements

• Home health skills checklist
• 1 supervisory signed reference from the past year of employment
• License verification
• Permanent address included in submission
• Requested time off must be included in submission

Additional Notes

• Traveler must carry auto liability insurance at the specified required limits in addition to proof of auto coverage
• Compliance file is due 3 Fridays before the Monday start date
• Mileage reimbursement is provided at the IRS standard rate
