Job Details

• Employment Type: Long-Term Assignment (LTA)
• Shift: Nights, 4 shifts per week, 10 hours each (9:00 PM - 7:30 AM)
• Contract Length: 13 weeks

Required Qualifications

• Valid CT Technologist license in Ohio
• Minimum of 1 year of CT Technologist experience, including experience within the last year
• BLS certification issued by the American Heart Association

Onboarding and Compliance Requirements

• Fit Test
• 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if TB positive (per regional guidelines)
• COVID-19 vaccine proof (including booster by Sept 2025), declination or exemption accepted except in CA and IL
• MMR vaccination proof (2 doses) or IgG titer lab report
• Hepatitis B vaccination series started prior to start date (titers not accepted)
• Varicella vaccination proof or IgG titer lab report
• Trinity 10 Panel Drug Screen completed within 30 days of start (includes methadone, phencyclidine, benzodiazepines, cocaine, propoxyphene, amphetamines, opiates, 6-acetyl morphine, methaqualone, barbiturates)
• Criminal background check (federal, nationwide, county for last 7 years) including SSN verification, dated within 30 days of start
• Education verification of highest academic degree or diploma/GED
• EPLS (Excluded Parties List System) report dated within 30 days
• OIG (Office of Inspector General) exclusion report dated within 30 days
• Confidentiality and proprietary information agreement signed
• Conflict of interest disclosure form signed
• Integrity and audit services form signed shortly before start
• Joint Commission CORE exam within the last year
• National Sex Offender Registry (NSO) check dated within 30 days
• System access form completed and uploaded one week before start
• Code of conduct form signed
• Standard I-9 completed by agency

Additional Information

• Resume and skills checklist (within one year) and two professional references required for submission
• Agency will provide the Trinity Skills Checklist for clinical contractors
• License/certification verification must be dated within 30 days of start and include online verification with machine-generated date
• All required documents must be uploaded into the vendor management system prior to onboarding
