<ul><li>Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.</li><li>Active Illinois Physical Therapist license required to start the assignment.</li><li>BLS certification from AHA or ARC may be required.</li><li>Home Health experience preferred.</li><li>Experience with OASIS and HCHB preferred.</li><li>Ability to perform in-home physical therapy evaluations and manage patient care across multiple counties.</li><li>Must be able to travel within coverage area counties: Clinton, Marion, Washington, Jefferson, Hamilton.</li><li>Minimum education requirement: Bachelor's Degree.</li></ul>

<ul><li>Facility type: Home Health.</li><li>Shift Name: Days.</li><li>Schedule: 4x8 (days).</li><li>Weekly hour guarantee: 32 hours per week.</li><li>On call/weekend requirement: No.</li><li>Mileage reimbursement between patient visits from first to last visit: Yes.</li><li>Coverage area includes multiple counties; candidate must be able to travel within the service area.</li><li>Required certification: Illinois PT license.</li></ul>
