Schedule
- Monday–Friday, 8:00 AM–4:30 PM
- 8 hours per day, 40 hours per week
Setting
- Inpatient Physical Rehab or LTAC facility
- Department includes Case Management, Case Manager, and/or Utilization Review
- Position covers both UR and discharge planning
Required Qualifications
- Valid RN license in Illinois
- Associate of Science in Nursing / Associate Degree in Nursing
- BLS issued by American Heart Association
- Case Management experience within the last year
- 2 years of Case Management experience
- Experience with discharge planning
- Experience with insurance authorization
- Experience with care coordination in an acute hospital setting
- Utilization Review experience
- Long Term Acute Care experience
Preferred Qualifications
- Healthcare setting experience
- Discharge planning experience
- Care coordination experience in an acute hospital setting
- Background in Long Term Acute Hospitals or Rehab
Key Responsibilities
- Collaborate with patients, families, and the multidisciplinary team
- Develop and manage plans of care from admission through discharge
- Manage throughput and length of stay
- Coordinate interdisciplinary rounding, care coordination, family meetings, utilization review, insurance authorization, concurrent reviews, advanced directives, surrogacy, discharge planning, DME, home health vs outpatient referrals, post-discharge referrals, transportation, and resources
- Set up vent facilities when needed
- Determine discharge pharmacy needs, including anticoagulants, IV antibiotics, and dialysis referrals
Patient Volume
- 13–25 patients per day
Additional Notes
- EPIC is used
- MCG is used for utilization review, but training can be provided
- No holidays required
- Weekend coverage may involve being available for questions
- Scrubs or business casual; no colors
- Orientation includes facility/regulatory requirements and may be shorter for clinicians with LTAC or Rehab experience
