Description

American Traveler is hiring a RN Case Manager for an acute care setting requiring an active IL RN license and 2+ years of case management or discharge planning experience.

Details

• Acute care setting within Inpatient Physical Rehab or Long-Term Acute Care (LTAC) units
• Case load of approximately 13-25 patients per day
• Handles both Utilization Review (UR) and discharge planning responsibilities
• Uses EPIC EMR system
• Works Monday through Friday, 8:00am to 4:30pm
• No holiday coverage required
• Possible flexible on-call availability for questions
• Interdisciplinary rounding schedule includes ICU Mondays and Med Surg Wednesdays for LTAC, with team conferences Monday through Friday for Rehab

Requirements

• Active IL RN license, LSW, or LCSW required
• BLS (AHA) certification required
• Minimum 2 years of experience required
• Healthcare setting experience preferred, including discharge planning and care coordination in an acute hospital setting
• Background in Long Term Acute Hospitals or Rehab preferred but not required
• Familiarity with MCG criteria helpful but not required (training provided)
• Strong computer skills including Microsoft software proficiency
• Strong analytical, problem-solving, and communication skills required
• Ability to multitask and prioritize in a fast-paced environment
• Basic understanding of acute and post-acute levels of care and resources required

Additional Information

• Manage complex patient populations including ventilator-dependent, airway management, IV antibiotics, respiratory, wound care, and chronic/acute illness cases in LTAC
• Support neuro, amputation, cardiac, trauma, and complex medical/surgical patients in Physical Rehab
• Collaborate with patients, families, and multidisciplinary teams to develop and manage individualized care plans from admission through discharge
• Coordinate resources including DME, home health, outpatient referrals, transportation, and post-discharge services
• Conduct utilization review, insurance authorizations, concurrent reviews, and family meetings
• Set up ventilator facilities and coordinate pharmacy/home infusion services as needed for discharge planning
• Facility-specific orientation provided, with additional regulatory training for candidates without LTAC or Rehab experience
• Dress code requires scrubs or business casual attire in neutral colors (no bright colors)
• Local candidates accepted with reduced rate applying to those within 50 miles of the facility
