Job Opportunity: Hospital Pharmacist (Day/Evening Rotating Weekends)

Location: Glenwood Springs, Colorado
Employment Type: Contract
Contract Length: 20 weeks


Job Summary

Seeking a licensed pharmacist to support a hospital during an EMR conversion from Meditech to Epic. The role involves rotating day and evening shifts, including rotating weekends, working 40 hours per week.


Schedule

  • Rotating day/evening shifts
  • Monday to Friday, starting at either 6:30 AM or 8:00 AM
  • Rotating weekends working 7:00 AM to 12:00 PM

Key Responsibilities

  • Prepare and dispense medication orders per physician requests
  • Maintain medication stock in patient care areas
  • Ensure safe, appropriate, and cost-effective medication therapies according to policies and protocols
  • Interpret medication orders and maintain computerized patient medication profiles
  • Compound and dispense pharmaceuticals, including IV admixtures, chemotherapy, and parenteral nutrition
  • Comply with federal, state, and local laws, including controlled substances handling and record-keeping
  • Monitor drug therapy regimens for contraindications, interactions, allergies, and dosing appropriateness
  • Perform pharmacokinetic drug dosing per hospital protocol
  • Review and interpret culture and sensitivity data for antibiotic appropriateness
  • Detect and report adverse drug reactions
  • Promote formulary compliance and therapeutic substitution protocols
  • Provide clinical consultation and drug information to medical staff, patients, and families
  • Document all clinical activities and interventions accurately
  • Participate in quality improvement and medication review activities
  • Collect data, conduct quality monitors and inspections, and maintain documentation
  • Assist in orientation, education, and training programs for pharmacy, nursing, and medical staff
  • Serve as a member of the Code Response Team

Qualifications

  • Active Colorado Pharmacist License (CO PHA) in good standing
  • Graduate of an ACPE-accredited School of Pharmacy with a BS Pharmacy degree
  • BLS and ACLS certification (American Heart Association)
  • Flu vaccine required (no exemptions accepted) during flu season (Oct 1 - Apr 1)
  • Epic experience required; Meditech experience a plus

  • Must be located at least 75 miles away from the facility (radius rule)
