Job Opportunity: Clinical Services Liaison - RN

Location: New York, NY
Contract Length: 9 weeks (61 days ≈ 9 weeks)

Job Description

  • Develop and maintain core therapy and service line admissions to home infusion through positive referral source relationships and responsive, caring customer service.
  • Identify and communicate opportunities for expanding the home infusion business among current and new referral sources.
  • Demonstrate thorough knowledge of core therapy referrals and qualifying criteria by payer source (e.g., TPN, Enteral Nutrition, Inotropic Agents for Medicare).
  • Evaluate current and future clinical and service needs for existing and new service areas.
  • Coordinate and/or conduct in-services and continuing education programs for referral sources and their support staff as appropriate.
  • Collaborate with referral sources (hospital case managers, home health agencies, physician offices) and home infusion staff to facilitate coordination of care for new patient referrals.
  • Coordinate and communicate critical workflow activities for new patient referrals, including intake, clinical operations (pharmacy and/or nursing), warehouse operations, and reimbursement as needed.
  • Document coordination of care activities, including services utilized, communications with patients and providers, and patient/caregiver education and training.
  • Ensure all documentation is legible, accurate, complete, and compliant with company policies and regulatory standards.
  • Maintain up-to-date referral source records and other requested records per policies and business goals.
  • Assist with activities to achieve favorable net revenue and gross profit goals.
  • Represent the home infusion pharmacy professionally and positively.
  • Attend and participate in office meetings, outreach meetings, and training programs.
  • Coordinate and cross-train departmental duties to maximize productivity and efficiency.
  • Stay current with developments in infusion therapy.
  • Comply with all company policies and directives.

Patient Care Responsibilities

  • Assess and evaluate patient care needs using critical thinking in patient care management.
  • Integrate relevant assessment and intervention skills in nursing care delivery.
  • Collaborate with interdisciplinary healthcare team in developing, implementing, and evaluating care plans.
  • Prioritize patient care aspects including teaching, rounding, coaching, planning post-hospital care, and delegating as appropriate.
  • Educate patients and caregivers on care plans, transitions, health promotion, and disease prevention.
  • Communicate effectively and professionally with patients, families, and the interdisciplinary care team.

Required Qualifications

  • Valid Registered Nurse (RN) license in New York.
  • Bachelor of Science in Nursing (BSN).
  • Minimum of 1 year of home health experience within the last year.
  • Experience in infusion therapy.
  • Current Basic Life Support (BLS) certification.
  • Proficient in venipuncture, urinary catheterization, and cardiopulmonary resuscitation (CPR).
