Job Details

• Shift Options:

  • Day shift, 3 shifts per week (12 hours each, 7:00 AM – 7:30 PM)
  • Day shift, 4 shifts per week (12 hours each, 7:00 AM – 7:30 PM)
• Hours per Week: 36 hours

Required Qualifications

• Valid Registered Nurse (RN) license in Kentucky
• Minimum 1 year of Emergency Department (ED) experience, with ED experience within the last year
• Certifications:

  • ACLS (American Heart Association)
  • BLS (American Heart Association)
  • PALS (American Heart Association)
  • TNCC (Trauma Nursing Core Course)
  • NIH Stroke Scale (NIHSS) Certification
  • CPI (Crisis Prevention Institute) Certification
• Education: Associate of Science in Nursing or Associate Degree in Nursing
• Orientation Safety Manual Acknowledgement

Health and Safety Requirements

• Physical Exam: Annual physical or physician note within 1 year prior to start, stating clearance to work without restrictions
• Immunizations and Testing:

  • Negative TB test (QuantiFERON blood test within 1 year; T-Spot/PPD not accepted)
  • TB Questionnaire if positive TB history
  • Proof of 2 MMR vaccinations or positive titers for Rubeola (Measles), Mumps, and Rubella
  • Hepatitis B series, titer, or declination (declination must be within 1 year)
  • Proof of 2 Varicella vaccinations or positive titer
  • Tetanus/Diphtheria/Tdap valid within 10 years (Td not accepted; declinations acceptable within 1 year)
  • Seasonal Flu Vaccination or approved medical/religious exemption (must be applied for within 48 hours of offer acceptance and approved prior to start during flu season)
• Drug Testing: Must be completed within 30 days of start using a specific 9-panel drug test (no THC) with expanded opiates and oxycodone; annual update required
• Fit Mask Test: Completed onsite; PAPRs not accommodated if facial hair present; medical or religious exemptions accepted but must be submitted on day one
• Latex Allergy: Facility accommodates latex allergies but requires notification prior to start

Documentation and Compliance

• Unexpired government-issued photo ID (Driver’s License or State ID)
• I9 Attestation completed by agency representative
• Signed and dated specialty job description, renewed annually
• Employee Health Attestation and completion of required health documentation uploads (TB results, TB risk assessment, OSHA respirator medical evaluation, flu vaccine/exemption during flu season)
• Annual skills checklist
• Kentucky Vulnerable Adult Maltreatment Registry verification within 30 days prior to start (annual update required)
