Physical Therapist (Outpatient) - Travel/Contract

Location: Cades, South Carolina
Contract Length: 13 weeks

Position Details

• Specialty: Physical Therapist (Outpatient)
• Shift: Days, Monday through Thursday 8:00 AM to 5:00 PM; Friday 8:00 AM to 12:00 PM
• Hours per week: 40
• Patient ratio: 1:1 (45 minutes per session)
• No weekends, on-call, or holiday shifts required
• No floating between units or facilities
• Facility type: Outpatient (not on hospital campus)
• Charting system: Epic
• Parking: Free

Required Qualifications

• Valid Physical Therapist license in South Carolina
• Minimum 1 year of Physical Therapist experience within the last year
• Experience in an outpatient setting
• Basic Life Support (BLS) certification (AHA or ARC only)

Compliance & Documentation Requirements

• Copy of Driver’s License and Social Security Card required
• Annual background check covering all locations lived/worked in past 7 years, including national and state criminal searches and sex offender registry
• Employment application/resume and proof of 2 clinical references (hospital evaluations accepted)
• Performance evaluation dated within 12 months of start
• Physical exam updated annually, attesting ability to meet physical demands of the position
• Immunizations and titers required:

  • Tetanus/Diphtheria/Tdap (within 10 years or declination form)
  • Measles (Rubeola), Mumps, Rubella (positive titer or 2 vaccines; boosters if low titer)
  • Hepatitis B (titer, vaccine series, or declination)
  • Varicella (positive titer or 2 vaccines; boosters if low titer)
• Negative TB test or chest X-ray within 90 days of start (2-step PPD or Quantiferon Gold; chest X-ray if positive)
• Annual TB questionnaire completion required
• 10-panel drug screen within 90 days of start (no instant tests)
• COVID vaccination not required but highly preferred; if declined, must complete vaccination declination form
• Flu vaccine required during flu season (Aug 1 - Mar 31); medical/religious exemptions accepted, declinations not accepted
• Respirator medical clearance annually for patient-facing roles (RespSafety questionnaire)

Facility-Specific Requirements

• Passport-style photo for badge (frontal view, solid light background)
• Completion of facility-specific orientation modules prior to start (approx. 2 hours)
• Signed agreements prior to start: Assignment Acknowledgment, Code of Conduct, Employee Confidentiality, Systems User Agreement, HIPAA Confidentiality Agreement
• User Access Request and Employee Info forms completed before start
• Upload COVID and flu vaccination documentation into facility system
• Travelers may be assigned to emergency response teams during hurricane season (June-November)
