ED - Emergency Department (1 year of experience)
ED - Emergency Department experience within the last year
BLS issued by American Heart Association
PALS issued by American Heart Association
ACLS issued by American Heart Association
NIHSS
Associate of Science in Nursing / Associate Degree in Nursing
Driver's License
Experience with Epic

MMR proof of vaccines or titers
Hepatitis B proof of vaccines or declination
Influenza vaccine, October 1 – March 31, with CHS declination accepted
TB test within the past 12 months prior to start date

Annual TB Questionnaire within the past 12 months
Physical and vision exam within the past 12 months prior to start date, including systems check, and required annually
12-panel drug screen with no THC within the past 12 months prior to start date
Criminal background check with 7-year criminal county search, SSN trace, and national criminal database within 12 months prior to start date
