Job Description

Patient Care Responsibilities

• Deliver patient care based on professional nursing standards, evidence-based practice, and organizational policies.
• Assess and evaluate patient needs, applying critical thinking in care management.
• Integrate assessment and intervention skills in nursing care delivery.
• Collaborate with interdisciplinary healthcare teams to develop, implement, and evaluate care plans.
• Prioritize patient care activities including teaching, rounding, coaching, discharge planning, and delegation.
• Educate patients and caregivers on care plans, health promotion, disease prevention, and transitions of care.
• Communicate effectively with patients, families, and healthcare team members.
• Manage assignments within a Modified Primary Nursing care delivery model and allocate resources efficiently.

Patient Experience

• Role model relationship-centered care through activities such as admission welcome, bedside shift report, purposeful hourly rounding, medication review, discharge wrap-up, and collaboration with physicians.
• Respect patient values, preferences, cultural diversity, and expressed needs.
• Establish and maintain therapeutic relationships with patients and families.
• Use evidence-based practices to understand and improve patient perceptions of care.
• Address patient and family concerns promptly and escalate as needed.

Operations

• Perform charge nurse duties as assigned.
• Coordinate with leadership to ensure adequate supplies and inventory.
• Escalate material or staffing shortages appropriately.
• Use patient-focused, cost-effective approaches to resource management.
• Document thoroughly in compliance with regulatory and legal standards.
• Facilitate patient throughput during shifts.

Qualifications

• Current New York State RN License
• BLS Certification
• Minimum 2 years of RN experience
• Minimum 2 years of rehabilitation and mechanical ventilation experience
• EPIC experience highly preferred
• Bachelor of Science in Nursing preferred; Associate degree considered with relevant experience or active BSN matriculation
• Travel experience (1 year) preferred for travel positions; not required for per diem

Work Schedule & Unit Information

• Schedule: 3 shifts per week, 12 hours each
• Average Unit Census: 21-25 beds per unit
