Job info

Develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals
Supervises licensed practical/vocational nurses as appropriate and assigns tasks according to state and federal regulations
Provides required supervisory visits
Completes comprehensive assessments, including medication reconciliation
Documents patient visits accurately and timely per policy and payer requirements
Performs initial and/or comprehensive nursing evaluation visits
Confirms home health eligibility and medical necessity
Determines primary focus of care and develops the plan of care within state-specific guidelines
Communicates patient care issues, visit assignments, schedule changes, orders, and OASIS data sets to coordinate care
Communicates with physicians, patients, and family members to support quality care and service excellence
