Required Qualifications

• Valid RN license in New York
• Med Surg / Telemetry (1 year of experience)
• Med Surg / Telemetry experience within the last year
• BLS - Basic Life Support issued by American Heart Association
• Associate of Science in Nursing / Associate Degree in Nursing
• Experience in Charge roles

Required Pre-Start Items

• Background check completed within 30 days of start and updated annually
• State and federal exclusion checks completed within 30 days of start
• Diploma or transcript for education verification
• Current skills checklist within the last year
• Facility-specific physical exam form completed by a physician within 30 days of start
• Fit test completed within 12 months of start
• Negative TB test or chest X-ray completed within 1 year of start
• Drug screen completed within 30 days of start
• Flu vaccination or documentation for the current season, if applicable
• COVID vaccine documentation is preferred

Notes

• A standard 9-panel drug screen is required
• Rapid drug tests are not allowed
• The physical exam cannot be completed onsite
