Employment Type: Travel/Contract
Schedule: Day shift, 5x8 hours, 8:00–16:30
Contract Length: 13 weeks

Provides biopsychosocial assessment, crisis intervention, short-term counseling, linkage with resources, and transition-of-care planning for patients and families/significant others of all ages
Provides psycho-education, may facilitate support groups, and consults on psychosocial aspects of care as part of the interdisciplinary treatment team
Serves as a liaison to community programs and collaborates with the multidisciplinary team on discharge planning
Supports patients in adjustment to illness and facilitates goals-of-care conversations
Evaluates patients for 5150 holds and arranges transport to the appropriate facility
Facilitates group therapy sessions as needed

Knowledge of Medicare, MediCal, and state/federal regulations is part of the role
Knowledge of child, elder, domestic, and dependent abuse reporting requirements is part of the role
