Case Manager

Overview

• Travel/contract role in Santa Rosa, California
• Start date: ASAP
• Contract length: 13 weeks
• Day shift, 5x8 hours, 8:00–16:30
• 40 hours per week
• First-time travelers accepted with clinical approval
• Locals accepted
• Floating may be required based on need
• Weekends required: 2 weekends every 6 weeks
• Call not required

Required Qualifications

• Valid RN license in California
• Case Management (2 years of experience)
• Case Management experience within the last year
• Associate of Science in Nursing / Associate Degree in Nursing

• Government-issued photo ID
• Social Security card, as applicable for I-9
• E-Verify attestation
• Background check
• Education verification
• Employment application covering at least the last 5 years
• Minimum of 2 references covering 1 year of employment total within the last 3 years in a comparable specialty/setting
• Skills checklist
• Physical exam within 1 year of start
• Fit mask record
• Tetanus/diphtheria/Tdap documentation or declination
• Negative TB test or chest X-ray
• Measles, mumps, rubella, hepatitis B, varicella, drug screen, flu, COVID, and respirator clearance documentation
