Job Opportunity: Physical Therapist - Home Health (Travel/Contract)

Job Description
- Position involves providing physical therapy services in a home health setting covering Hesperia, Victorville, and the High Desert area.
- Home Health experience preferred.
- California PT license preferred; candidates with pending CA license who have applied may be considered.
- OASIS experience required; candidates without OASIS experience may be considered if they have solid home health experience.
- Guaranteed hours based on completed visits: 25 visits equate to 40 hours.
- COVID exemptions permitted.
- Mileage reimbursement follows IRS standard rates.
Required Credentials & Certifications
- Licenses: California PT license (or pending with application submitted)
- Certifications:
- BLS (Basic Life Support) - AHA only, instructor or provider acceptable (required for all)
- ACLS, PALS, NRP, EFM as applicable (AHA only)
- State License verification (primary source online verification required)
- NURSYS verification for RN, LPN/LVN, AP as applicable
- National Tech Cert Verification as applicable for allied professionals
Required Documents & Compliance
- Government-issued photo ID (Driver’s License or State ID)
- Valid passport copy before start
- Background checks including 7-year search, EPLS/GSA/SAM, HHS/OIG (updated annually)
- Photo for badge (passport style, shoulders up, front facing, plain background)
- Personal or auto insurance with current expiration dates
- Auto registration with current expiration dates
- Facility-specific training and policy acknowledgments (e.g., hand hygiene)
- Physical exam within 12 months of start
- Immunizations and health screenings:
- TDAP vaccine within past 10 years (no declinations accepted)
- Negative TB test or chest X-ray within 1 year (with special rules for past positives)
- Proof of immunity or vaccination for Measles, Mumps, Rubella (for those born 1957 or later)
- Hepatitis B titer or vaccine series (declination accepted if within 1 year)
- Varicella immunity or vaccination (no declinations accepted)
- Negative 9-panel drug screen within 1 year
- Seasonal flu vaccination/documentation (declinations accepted)
- COVID-19 vaccination or approved medical/religious exemption, including booster (required)
Competency Requirements
- Medication Test/Pharmacology Competency Exam (as applicable)
- Competency Exam with minimum passing score of 80% or higher if Angoff score applies
- Annual skills checklist

- Two professional references required
