Work Schedule & Requirements

• 36 hours per week
• Availability every other weekend
• Availability required on Thanksgiving and Christmas Eve/Day
• Must hold an active RN license

Required Credentials & Certifications

• BLS (Basic Life Support): Must be American Heart Association (AHA) certified
• Nursys Verification: Completed within 30 days prior to start; disciplinary actions require additional documentation
• License/Certificate Verification: Current license and primary source verification within 30 days prior to start
• Ascension Color Vision Test: Completed prior to start and updated every 3 years
• Ascension Core Competencies: HIPAA, Joint Commission NPSG, OSHA topics; completed within a year of start and updated yearly (minimum 80% score, max 2 attempts)
• NIH Certification: Group A updated yearly; Groups B-F updated every 2 years
• Ascension EKG Exam: Must pass with at least 80%, max 2 attempts; updated as new editions are released
• Ascension Onboarding Requirements Education Attestation: Completion of pre-hire education attestation required
• Ascension Nursing Contract Worker Onboarding: Completed onsite prior to start
• Ascension Nova StatStrip: Pre-hire modules required for non-allied positions
• Ascension Cerner Test-Out: Cerner test results and training checklist with signed attestation
• Ascension Badge Photo: Forward-facing headshot with white or blue background, no alterations
• Ascension Skills Checklist: Clinical skills checklist completed within one year of start and updated annually

Health & Immunization Requirements

• MMR: Documented proof of two vaccines or positive IGG titer; no declinations accepted
• Fit Test: Required for all travelers; preferred prior to start, can be completed onsite; updated annually
• Hepatitis B: Documentation of vaccination series completion or signed declination
• Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement
• Positive PPD / Negative Chest X-Ray and Annual TB Questionnaire: Optional but recommended; must provide documentation if positive
• 2 Step PPD OR IGRA (Quantiferon/Tspot): Required within 30 days prior to start; updated annually
• Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider; no declinations accepted
• Tdap: Valid within 10 years; no declinations accepted
• Health Statement: Statement of good health or physical exam prior to first assignment, updated annually
• Ascension 12 Panel Drug Screen: Completed within 30 days prior to start and updated annually; must include all 12 drugs tested with MRO review if applicable

Professional & Background Verification

• Driver's License/State ID: Must be current and match address requirements
• Ascension Profile: Resume or professional profile required
• Ascension 2 Professional References: At least two professional references prior to first assignment
• Ascension Criminal Background Check: Includes Social Security Trace, Employment Verification, OIG Search, National Sex Offender; updated annually or after break in service > 90 days
• Ascension EPLS (Excluded Parties List System): Run within 90 days of start; rerun if break in service > 90 days
• Ascension Education Verification: Verification of education relevant to clinical work within 90 days of start; updated after break in service > 90 days
• Ascension Confirmation of Assignment: Provided upon initial assignment and updated with extensions
• Ascension Employment Verification: Third-party employment verification for past 7 years; updated annually or after break in service > 90 days

Optional / Additional Requirements

• Ascension Unit Specific Checklist: Optional for onboarding, must be uploaded by unit leader within a week
• Hiring Manager Evaluation: Post-assignment evaluation (optional)
• Ascension Name Change Document: Legal documentation of name change (optional)
