Onboarding Requirements (Required for Onboard)
- Ascension Cerner Test-Out (Cerner test results and training checklist with signed attestation)
- Ascension Badge Photo (forward-facing headshot with white or blue background, no alterations)
- Health Statement (statement of good health or completed physical exam prior to first assignment, updated annually)
- Fit Test (required for all travelers; preferred prior to start, can be completed onsite)
- MMR (proof of two vaccines or positive IGG titer; no declinations accepted)
- Hepatitis B (vaccination series completion or signed declination)
- Varicella (proof of two vaccines, serological immunity, or verified history; no declinations accepted)
- Tdap (valid within 10 years; no declinations accepted)
- 2 Step PPD or IGRA (TB testing with required timing and documentation)
- Positive PPD requires negative chest X-ray and annual TB questionnaire
- Influenza Vaccine (optional; flu season Oct 1 - Mar 31; declinations accepted with mask requirement)
- Ascension 12 Panel Drug Screen (within 30 days prior to start, updated annually)
- Ascension Criminal Background Check (including SSN trace, employment verification, OIG search, national sex offender check)
- Ascension EPLS (Exclusionary Party List System) check within 90 days of start
- Ascension Education Verification (within 90 days of start; official transcripts if verification fails)
- Ascension Employment Verification (past 7 years, updated annually)
- Ascension Confirmation of Assignment (agency letterhead with start/end dates)
- BLS Certification (AHA required)
- ACLS Certification (AHA required)
- Ascension Specialty Exam (minimum 80% passing score, updated every 3 years)
- Ascension Core Competencies (HIPAA, Joint Commission, OSHA topics; minimum 80% score, updated yearly)
- Ascension EKG Exam (minimum 80% passing score, updated as required)
- Ascension Color Vision Test (updated every 3 years)
- NIH Certification (Group A yearly, Groups B-F every 2 years)
- CPI/MOAB High Risk Online Portion (completed prior to start)
- Ascension Onboarding Requirements Education Attestation
