RN Medical/Surgical - Wisconsin (Mequon)

Job Details

• Employment Type: Long-Term Assignment (LTA)
• Contract Length: 13 weeks
• Shift: 7:00 PM – 7:30 AM (Night Shift)
• Location: Must be willing to float onsite within skill set or to sites within 40 miles if needed
• Unit: Telemetry background required (must be able to read telemetry)

Required Experience & Skills

• Minimum 2 years of Medical/Surgical nursing experience
• Prior travel experience required
• Experience with EPIC electronic health record system
• Telemetry experience required

Required Qualifications & Certifications

• Current, valid RN license for Wisconsin (Nursys verification within 30 days prior to start)
• BLS certification (American Heart Association)
• NIH certification (updated as required)
• Ascension Specialty Exam (minimum 80% passing score, updated every 3 years)
• Ascension Core Competencies (including HIPAA, Joint Commission, OSHA topics; minimum 80% passing score)
• Ascension EKG Exam (minimum 80% passing score)
• Ascension Color Vision test (updated every 3 years)
• Ascension 12 Panel Drug Screen (within 30 days prior to start, updated annually)
• Ascension Skills Checklist (clinical specialty competency, updated annually)
• Ascension Education Verification (within 90 days of start)
• Ascension Employment Verification (past 7 years, updated annually)
• Ascension Criminal Background Check (including SSN Trace, OIG Search, National Sex Offender, updated annually)
• Ascension EPLS (Exclusionary Party List System) check (within 90 days of start)
• Ascension Onboarding Requirements Education Attestation
• Ascension Nursing Contract Worker Onboarding (completed onsite)
• Ascension Nova StatStrip training (pre-hire modules)
• Ascension Epic Test-Out (training and test results required)
• Ascension Profile (resume or professional profile)
• Ascension 2 Professional References (at least two professional references)
• Ascension Confirmation of Assignment (agency letterhead documentation)
• Ascension Signed Assignment Agreement
• Ascension Badge Photo (forward-facing headshot with white or blue background, no filters)
• Ascension Name Change Document (if applicable)

Health & Immunization Requirements

• MMR: Documented proof of two vaccines or positive IGG titer; no declinations accepted
• Varicella: Documented proof of two vaccines, serological immunity, or history verified by healthcare provider; no declinations accepted
• Tdap: Valid within 10 years; no declinations accepted
• Hepatitis B: Documentation of vaccination series completion or signed declination
• Influenza Vaccine: Optional; if declined, mask required during flu season (Oct 1 – Mar 31)
• Fit Test: Required annually; specific mask types required for WI
• TB Testing: Positive IGRA or PPD results, chest X-ray within one year showing no infection, and annual TB questionnaire
• Health Statement: Statement of good health or physical exam prior to first assignment, updated annually
• 2 Step PPD or IGRA within 90 days of start

Additional Requirements

• No artificial or acrylic nails; nails must be short (< 1/4 inch)
• Must be able to float within skill set or to other sites within 40 miles if needed

This position requires a dedicated RN with solid medical/surgical and telemetry experience, who is comfortable working night shifts and traveling within the region. Compliance with extensive credentialing, health, and onboarding requirements is mandatory.
