Pharmacist Outpatient

Overview

• Schedule: Days, Monday–Friday, 8:00–17:00, with 1 hour lunch
• Hours: 40 per week
• Onsite role seeing 14 patients per day

Required Qualifications

• Doctorate of Pharmacy or equivalent from an accredited College of Pharmacy
• 2-year ASHP residency in HIV
• 5 years of direct HIV patient care demonstrating medication management
• Texas State Board of Pharmacy license or eligibility for the same
• BLS

Role Summary

• Clinical Pharmacist Disease Specialist for Amb HIV Hep C
• Provides clinical leadership for pharmaceutical care
• Serves as a resource for outpatient and inpatient clinicians for HIV and/or Hepatitis pharmacotherapy
• Works with the Inpatient Infectious Diseases Clinical Pharmacist to transition HIV patients from the hospital setting to the HIV Clinic on appropriate therapy
• Develops and optimizes pharmacotherapeutic regimens through collaborative practice agreements
• Performs drug utilization reviews and monitors for drug interactions and potential adverse events
• Works to minimize polypharmacy and develops patient medication action plans with self-management goals
• Precepts pharmacy students or residents and participates in didactic and experiential education
• Participates as a key component of the multidisciplinary patient care team in the Medical Home
