Requirements

• Licensure: NY LPN license in hand
• Experience: Hospital experience within the last 2 years
• Independence: Must be able to take own patient assignments with RN support
• Prior Travel Experience: Not explicitly required

• ACLS (AHA Provider/Instructor/RQI acceptable)
• BLS (AHA Provider/Instructor/RQI acceptable)
• PALS (AHA Provider/Instructor/RQI acceptable)
• NRP (Neonatal Resuscitation Procedure)
• TNCC (Trauma Nursing Core Course)
• EFM (Electronic Fetal Monitoring)
• State License Verification (Primary Source Verification within 30 days of start)
• NURSYS Verification (for LPN/LVN within 30 days of start)
• Orientation Safety Manual Acknowledgement
• Tech Cert Verification (if applicable within 30 days of start)

Background Checks & Verifications

• 7-year criminal background check (felony checks last 7 years) within 3 months of start
• SSN Trace, National Criminal, National Sex Offender checks
• Education verification from primary source (hard copy degrees not accepted)
• EPLS/GSA/SAM and HHS/OIG checks within 3 months of start and monthly thereafter
• Foreign equivalency education evaluation if applicable
• Transcript/Diploma if no education verification on file
• Work history verification (at least 2 past employments verified)
• References (2 references within 3 years required for Aya candidates)

Health & Compliance Requirements (Must be completed within 5 days of signing contract)

• Physical exam (within 1 year, updated annually)
• Fit mask test (within 1 year, updated annually; acceptable masks include Halyard duckbill 1870+1860 Regular or 1860 Small; PAPR accepted if failed)
• Negative TB test or chest X-ray (within 1 year)
• TB questionnaire (required in lieu of annual TB test renewal)
• Immunizations:

  • Rubeola (Measles): Positive titer or 2 vaccines (age exemption if born before 1957)
  • Mumps: Positive titer or 2 vaccines (age exemption if born before 1957)
  • Rubella: Positive titer or 1 MMR/Rubella vaccine (age exemption if born before 1957)
  • Hepatitis B: Positive titer, completed series, or declination (additional requirements for dialysis)
  • Varicella: Positive titer, 2 vaccines, or documented history
• Drug screening (within 30 days of start; includes amphetamines, cocaine, ecstasy, fentanyl, heroin, opiates)
• Flu vaccination or signed declination during flu season (documentation deadline December 3)
• Eye exam (Snellen, within 1 year)
• COVID vaccination copy preferred but not required
