<h3> Title: Licensed Medical Social Worker (LMSW) – Hospice Specialty: Hospice Care Schedule: Monday – Friday Hours: 25 patient-facing hours weekly / 40 hours per week Location: New Hampshire (Territory-based)</h3> Position Overview We are seeking an experienced Licensed Medical Social Worker (LMSW) with recent hospice experience to provide compassionate support and care coordination for geriatric patients in a home hospice setting. The ideal candidate will be comfortable traveling throughout the assigned territory, maintaining timely documentation, and collaborating with the interdisciplinary care team. Schedule &amp; Caseload <ul> 	<li> Monday – Friday</li> 	<li> 40 hours per week</li> 	<li> Approximately 5–7 patient visits per day (please confirm during interview)</li> 	<li> No weekends</li> 	<li> No on-call requirements</li> </ul> Required Qualifications <ul> 	<li> Active New Hampshire LMSW license in hand</li> 	<li> Minimum 1 year of recent Hospice Social Work experience</li> 	<li> Experience with HCHB (Homecare Homebase) EMR required</li> 	<li> BLS Certification required</li> 	<li> Strong documentation and communication skills</li> 	<li> Must be flexible with assigned territory coverage</li> </ul> Patient Population <ul> 	<li> Primarily Geriatric Hospice Patients</li> </ul> Territory <ul> 	<li> Coverage area may extend up to a 60-mile radius from the assigned office/hub</li> 	<li> Traveler must be willing to float within the designated territory as needed</li> </ul>
