Job Opportunity: Licensed Masters of Social Worker (LMSW)

Position Details

• Specialty: Hospice Social Services (experience in Hospice required)
• Employment Type: Travel/Contract
• Contract Length: 13 weeks
• Schedule: Monday to Friday, approximately 24-25 hours per week (3 shifts of 8 hours, 8:00 AM to 1:00 PM)
• Average Patients per Day: 5-7 (geriatric cases)
• On-Call: No
• Weekends: No
• Dress Code: Scrubs (any color), no jeans
• Territory: Up to a 60-mile radius from the office; flexibility with territory expected
• Technology Provided: iPad (cell phone use not allowed)
• Mileage Reimbursed at IRS standard rate
• Locals accepted at travel rate
• Timely documentation and communication required

Required Qualifications

• Minimum 1 year recent experience in Hospice Social Services
• Experience with HCHB Electronic Medical Record (EMR) system required
• BLS (Basic Life Support) certification required (AHA, ARC, Military acceptable)
• Valid license to practice LMSW in the state of New Hampshire (license must be in hand)
• First-time travelers not accepted (prior travel experience required)

Background and Compliance Requirements

• Comprehensive background checks required within 90 days of start, including but not limited to:

  • SSN Trace
  • County searches for last 7 years of residence/work/school
  • FACIS Level 1
  • Nationwide Sex Offender Registry
  • Motor Vehicle Records (MVR)
  • Education and Work History Verification
  • State-specific searches as applicable (e.g., New Hampshire state police search)
• EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
• Fingerprinting may be required depending on state placement; must be current and cleared
• Education verification of highest degree related to profession required
• Employment history verification covering 7 years required, with no gaps
• Two professional references required within 2 years of start

Health and Safety Requirements

• Physical exam within the last 12 months (not required annually)
• Negative TB test or chest X-ray within required timeframes; annual TB questionnaire required
• Immunizations or proof of immunity required for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• 9-panel drug screen within 12 months of start (instant acceptable)
• Flu seasonal vaccination or facility-specific declination form required during flu season
• COVID vaccination or facility-specific declination required
• Annual safety manual acknowledgment (Joint Commission Training Safety Manual)

Additional Facility-Specific Requirements

• Proof of current auto insurance required if job responsibilities include driving
• Completion of Acceptable Use Policy prior to start
• Facility-specific flu and COVID statement form required if declining flu vaccine
• Agency background attestation required before start

This position is ideal for experienced hospice social workers with prior travel contract experience who are flexible with travel territory and committed to compliance and timely documentation.
