Job Opportunity: RN (Registered Nurse) - Case Manager

Location: White Plains, New York
Contract Length: 13 weeks (travel assignment)
Shift: Days
Hours: 10 hours/day, approximately 38 hours/week

Job Summary

The RN Care Manager is responsible for performing comprehensive initial assessments on admission, utilizing established tools to identify high-risk patients and ensure appropriate supportive services for discharge. The role includes ongoing reassessment, utilization review (UR) activities, and multidisciplinary care coordination to reduce length of stay (LOS), prevent readmissions, and improve patient satisfaction.

Key Responsibilities

• Conduct comprehensive initial patient assessments and ongoing reassessments throughout hospital stay.
• Coordinate multidisciplinary care at both intra- and inter-hospital levels.
• Manage transitional planning including patient/caregiver education, aftercare arrangements, and commercial payer involvement.
• Collaborate with healthcare professionals to evaluate needs and ensure safe discharge, especially for geriatric patients.
• Perform utilization review activities such as commercial payer reviews, disposition determination, audits, and clinical admission reviews.
• Document all activities per department requirements.
• Participate in performance improvement initiatives.

Required Qualifications

• Valid Registered Nurse (RN) license in New York State
• Minimum of 2 years of case management experience, including experience within the last year
• Basic Life Support (BLS) certification issued by the American Heart Association (AHA)
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Discharge planning experience
• Utilization Review experience, including familiarity with InterQual criteria

Preferred Qualifications

• Care Management certification (e.g., CMC, CCM).
• Screen certification.
• Minimum 3 years clinical experience in Emergency Department, Critical Care, or Medical/Surgical settings.
• Previous case management experience.
• Knowledge of healthcare financial, regulatory, and payer issues.
• Familiarity with state, local, and federal healthcare programs.
• Experience using Milliman/InterQual criteria.
• Strong computer skills.

Additional Information

• Parking reimbursement available at a minimum of $1.25 per hour.
• Work schedule consists of four 10-hour shifts per week.
