Physical Therapist – Outpatient

Location: Salida, Colorado
Employment Type: Contract
Contract Length: 13 weeks
Shift: Monday to Thursday, 7:00 AM – 5:00 PM (9-hour days) with an hour lunch; Fridays and weekends off
Hours per Week: 36

Job Description

• Work in a rural healthcare outpatient clinic setting.
• Provide physical therapy services within the scope of practice.
• Conduct thorough patient assessments to determine diagnosis, prognosis, and intervention strategies.
• Design and modify therapeutic plans to help patients regain daily function.
• Focus on injury and disability prevention by promoting health, fitness, and wellness for all ages.
• Participate in consultation, education, and research activities related to physical therapy.
• Maintain accurate and up-to-date patient records, including physician orders, treatment plans, progress notes, and discharge summaries.

Required Qualifications

• Valid Physical Therapist license in Colorado.
• Minimum 2 years of experience as a Physical Therapist, including orthopedic care.
• Experience working in an outpatient setting within the last year.
• Current Basic Life Support (BLS) certification.

Additional Information

• Training or certifications such as ASHA, CHT, or ACP are noted but not required.
