Job Opportunity: Speech-Language Pathologist (Home Health)

Location: San Jose, California
Employment Type: Travel/Contract
Contract Length: 4 months (minimum for less experienced clinicians is 6 months, non-negotiable)


Job Description

  • Provide home health therapy services primarily in counties including Alameda, San Joaquin/Stanilaus, Santa Clara, Sonoma, and Marin.
  • Patient caseload focuses on home health, wound care, and geriatric patients.
  • Minimum of 23-24 patient visits per week required to qualify for a 40-hour workweek guarantee.
  • No weekend shifts required.
  • Visits and charting are done during the workday; charting must be completed onsite using provided tablets (no work from home).
  • Mileage reimbursed for travel between patients (not to/from home).
  • Dress code: Any color scrubs and closed-toe shoes.
  • Free residential parking available.

Required Qualifications

  • Valid Speech Language Pathologist license in California.
  • Minimum of 1 year of Speech Language Pathologist experience within the last year.
  • Experience in a Home Health setting.
  • Basic Life Support (BLS) certification required at time of submission.

Schedule

  • Day shift, Monday through Friday, 8:00 AM to 5:00 PM (5 days x 8 hours = 40 hours/week).

Compliance and Documentation Requirements

Candidates must provide the following before starting:

  • Government-issued photo ID (Driver’s License or State ID).
  • Emergency contact information.
  • Background checks within 1 year, including SS trace, county searches (7 years), nationwide, and sex offender registry. Annual EPLS/GSA/SAM and HHS/OIG checks required.
  • Certifications: BLS (required), ACLS and PALS preferred but not mandatory.
  • State license primary source verification.
  • Physical exam within 6 months signed by MD/DO confirming freedom from active communicable diseases.
  • Immunizations/vaccinations or documented exemptions for:
    • COVID-19 (vaccine or medical/religious exemption)
    • Influenza (vaccine or declination form during flu season)
    • Tetanus/Diphtheria/Tdap (within 10 years or exemption)
    • Measles, Mumps, Rubella (proof of immunity or vaccination as per birth year guidelines)
    • Hepatitis B (titer, vaccine series, or declination)
    • Varicella (titer, vaccine, history of chickenpox, or declination)
  • Negative TB test or chest X-ray within 1 year (with special rules for past positives).
  • 9-panel drug screen within 1 year of start (instant drug screen acceptable).
  • Auto insurance card.
  • Facility-specific influenza declination form if opting out of flu vaccine.
  • Annual skills checklist and competency exams as applicable.
  • Two professional references required for travel or allied care clinicians (within 3 years).

Additional Notes

  • Territory and county assignments will be discussed during the interview; flexibility to cover surrounding counties may be required.
  • Locals within a 50-mile radius are accepted.
  • All required documents must be submitted at time of application; no additional documents accepted after offer.
  • Compliance processing may take 2-3 weeks before start date.
