Job Opportunity: Case Manager (Registered Nurse)

Location: Santa Rosa, California
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Description

  • Coordinate utilization management, resource management, discharge planning, post-acute care referrals, and care facilitation.
  • Manage acute patient populations across the care continuum to provide coordinated and integrated care.
  • Focus on preventing unnecessary admissions or readmissions.
  • Provide discharge planning coordination and intervention for high-risk patients presenting to the Emergency Department.
  • Promote patient wellness, improved care outcomes, efficient utilization of health services, and minimize payment denials for patients with complex health needs.

Shift Details

  • Day shift, 5 days a week, 8-hour shifts (08:30 AM - 5:00 PM).
  • Minimum 2 weekend shifts per month.
  • If floated, work will be limited to Case Management only.

Required Qualifications

  • Valid Registered Nurse (RN) license in California.
  • Minimum 2 years of Case Management experience, including experience within the last year.
  • Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross.
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).

Background and Compliance Requirements

  • 7-year background check required within 6 months of start, including social security trace with all aliases, county searches, nationwide sex offender search, education verification, and FACIS Level 3 screening.
  • EPLS/GSA/SAM and HHS/OIG checks required and updated annually.
  • Education verification required; Bachelor of Science in Nursing preferred.
  • Drug screening: Negative 9-panel drug screen within 6 months of start (excluding marijuana due to CA law).
  • Physical exam required within 1 year of start, including clearance for work.
  • Fit mask test required within 1 year of start and must be renewed annually (exceptions apply).
  • Immunizations and titers required:
    • Tetanus/Diphtheria/Tdap (within 10 years)
    • Negative TB test or chest X-ray within 90 days of start, renewed annually
    • Measles (Rubeola), Mumps, Rubella, Varicella immunity or vaccination as required
    • Hepatitis B series or declination
    • Seasonal flu vaccination during flu season (Nov 1 - Apr 30) or signed declination
  • Respiratory safety clearance required prior to start and renewed annually (exceptions apply).

Documentation and Verification

  • Must provide full 7 years of work history with no gaps, including city and state of employment.
  • Employment verification required for up to 7 years; gaps over 30 days must be explained.
  • Two professional references required within 3 years for clinical positions.
  • Unit-specific skills checklist required and must be renewed annually.
  • State license primary source verification required within 30 days of start.
  • NURSYS verification required within 30 days for RN license.
  • Official transcripts/diplomas or attestation of verification attempts required for education verification.

Facility-Specific Requirements

  • Candidate must review and sign facility-specific documents prior to start, including:
    • Job description acknowledgment
    • Standards of Business Conduct
    • Code of Conduct
    • Vendor training (2024 or 2025 depending on contract date)
    • Overpayment refund policy
    • General regulatory orientation for contracted workers
  • TB screening questionnaire required within 90 days of start and renewed annually.
  • Religious exemption forms required for PAPR use, facial hair, and certain vaccine exemptions; approvals handled on a case-by-case basis.
  • Photo for badge required for per diem positions.
