Required Credentials and Certifications

Licenses & Certifications

  • BLS (Basic Life Support): Must be AHA certified.
  • Nursys Verification: Within 30 days prior to start; disciplinary actions require FACIS Questionnaire.
  • License/Certificate Verification: Current license and primary source verification within 30 days prior to start.
  • NIH Certification: Group A updated yearly; Groups B-F updated every 2 years.
  • MOAB Moderate Risk De-Escalation Training: Required.
  • Ascension Core Competencies: HIPAA, Joint Commission NPSG, OSHA topics; completed within a year of start and updated yearly with minimum 80% score.
  • Ascension EKG Exam: Must pass with at least 80%, two attempts allowed.
  • Ascension Cerner Test-Out: Required test results and training checklist with signed attestation.

Personal Health History & Immunizations

  • Ascension Color Vision: Completed prior to start and updated every 3 years.
  • MMR: Documented proof of two vaccines or positive IGG titer; no declinations accepted.
  • Fit Test: Required for all travelers; preferred prior to start, updated annually.
  • Hepatitis B: Documentation of vaccination series completion or signed declination.
  • Influenza Vaccine: Required for flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement.
  • Positive PPD/Negative Chest X-Ray and Annual TB Questionnaire: Must provide positive IGRA or PPD results, chest x-ray within a year, and completed TB questionnaire.
  • 2 Step PPD OR IGRA (Quantiferon/Tspot): Within 1 year and 30 days of start; updated annually.
  • Varicella: Proof of two vaccines, immunity, or history; no declinations accepted.
  • Tdap: Valid for 10 years; no declinations accepted.
  • Health Statement: Statement of good health or physical exam prior to first assignment, updated annually.
  • Ascension 12 Panel Drug Screen: Within 30 days prior to start; includes testing for 12 specified drugs; updated annually.

Professional & Onboarding Requirements

  • Ascension Onboarding Requirements Education Attestation: Completion of pre-hire education attestation.
  • Ascension Nursing Contract Worker Onboarding: Completed onsite prior to start.
  • Ascension Nova StatStrip: Pre-hire modules required for non-allied positions.
  • Driver's License/State ID: Must be current, match address on file, and comply with compact license rules.
  • Ascension Profile: Resume or professional profile required.
  • Ascension 2 Professional References: At least two professional references prior to first assignment.
  • Ascension Criminal Background Check: Includes SSN trace, employment verification, OIG search, and national sex offender check; updated annually or after break in service > 90 days.
  • Ascension EPLS: Run within 90 days of start; rerun if break in service > 90 days.
  • Ascension Skills Checklist: Clinical skills checklist completed within one year of start; updated annually.
  • Ascension Education Verification: Verification of education relevant to clinical work within 90 days of start; rerun if break in service > 90 days.
  • Ascension Confirmation of Assignment: Provided upon initial assignment and updated with extensions.
  • Ascension Employment Verification: Third-party employment verification for past 7 years; updated annually or after break in service > 90 days.

Optional Credentials

  • Ascension Unit Specific Checklist: Optional for onboarding; must be uploaded within a week by unit leader.
