Required Credentials & Documentation

• License & Verification:

  • Current nursing license with primary source verification within 30 days prior to assignment start.
  • Nursys verification required within 30 days prior to start.
  • Driver’s License or State ID (must be current and match address on file).

• Certifications:

  • BLS (American Heart Association required)
  • NIH Certification (updated yearly or every 2 years depending on group)
  • MOAB Moderate Risk De-Escalation Training
  • Tdap (valid for 10 years, no declinations accepted)

• Health & Immunizations:

  • MMR: Proof of two vaccines or positive IGG titer (no declinations accepted).
  • Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider (no declinations accepted).
  • Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement.
  • Hepatitis B: Documentation of vaccination series completion or signed declination.
  • 2 Step PPD or IGRA (Quantiferon/Tspot): Recent testing required with annual TB questionnaire.
  • Positive PPD requires negative chest X-ray within one year and TB questionnaire.
  • Fit Test: Required for all travelers, preferably completed prior to start; updated annually.
  • Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually.

• Background & Screening:

  • Criminal Background Check including Social Security Trace, Employment Verification, OIG Search, National Sex Offender Registry, and others within 30 days of assignment start; updated annually or after breaks in service.
  • EPLS check within 90 days of start; rerun if break in service > 90 days.
  • 12 Panel Drug Screen within 30 days prior to start; updated annually or after breaks in service.

• Onboarding & Competency Requirements:

  • Signed Assignment Agreement (required for onboarding).
  • Ascension Resume/Profile and at least two professional references (required for submission).
  • Ascension Core Competencies: HIPAA, Joint Commission NPSG, OSHA topics; minimum 80% score, two attempts allowed.
  • Ascension EKG Exam: Passing score of 80% required, two attempts allowed.
  • Ascension Cerner Test-Out: Test results and training checklist with signed attestation required.
  • Ascension Skills Checklist: Clinical skills checklist to be completed within one year of start and updated annually.
  • Ascension Onboarding Education Attestation and Nursing Contract Worker Onboarding (completed onsite).
  • Ascension Nova StatStrip pre-hire modules (required for non-allied positions).
  • Ascension Employment and Education Verification: Employment verification for past 7 years and education verification within 90 days of start.
  • Confirmation of Assignment documentation upon initial assignment and extensions.

Optional Credentials

• Ascension Unit Specific Checklist (optional for onboarding, uploaded by unit leader).
• Positive PPD documentation and annual TB questionnaire updates.
• Hiring Manager Evaluation (post-assignment).
• Legal Name Change Documentation.

This position requires thorough credentialing and compliance with Ascension Health’s onboarding and ongoing requirements to ensure patient safety and regulatory adherence.
