Job Opportunity: Respiratory Therapist

Location: Des Moines, Iowa
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks (approximately 3 months)
Schedule: Every Other Week (EOW)

Required Certifications and Licenses

• BLS (American Heart Association certification required for all direct patient care personnel)
• ACLS (required for RNs in critical care areas)
• RRT or CRT credential
• Valid Respiratory Therapist license (verification must be dated within 30 days of start)

Onboarding and Compliance Requirements

• Trinity Code of Conduct Form
• Trinity I9 Form
• Confidentiality and System Security Agreement
• Integrity and Audit Services Form
• Non-Employee Upload Form (specific to Iowa)
• Specialty Specific Competency Testing (exam within last year)
• Profile submission including resume, skills checklist (within a year), and 2 professional references
• Conflict of Interest Disclosure Form
• Criminal Background Check (federal, nationwide, county for last 7 years, including SSN verification)
