Registered Nurse Case Manager - Contract

Location: Glenwood Springs, Colorado
Employment Type: Contract
Contract Length: 13 weeks

Job Summary

Provide case management services to inpatients and outpatients, assisting with the overall management of their plan of care across the continuum. This is a full-time day position with weekend rotation and no on-call duties.

Schedule

3 shifts per week
12-hour shifts
Weekend rotation

Required Qualifications

Graduate of an accredited or state board-approved Registered Nursing program
Current Registered Nurse (RN) license in good standing
Minimum of 3 years RN Case Management experience (highest required years of experience)
EPIC electronic health record experience is required
Flu vaccine required (no exemptions)

Additional Information

Potential housing available: furnished studio and one-bedroom apartments near the facility, subject to availability
Travel candidates must have a permanent address at least 75 miles from the facility
