Registered Nurse - Home Care

Location: Barre, Vermont
Employment Type: Contract
Contract Length: 16 weeks
Shift: Days (Monday to Friday, some weekend coverage)
Hours per Week: 36

Job Summary

Provide compassionate, personalized end-of-life care in patients' homes, focusing on physical and emotional well-being. Deliver expert pain and symptom management while collaborating with an interdisciplinary team, local physicians, and community providers to optimize patient quality of life.

Required Qualifications

• Valid RN license in Vermont
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Minimum of 1 year of recent Home Health experience within the last year
• Hospice experience
• Experience with OASIS
• Basic Life Support (BLS) certification

Key Responsibilities

• Provide compassionate hospice care to patients and support to their families
• Manage pain and symptoms effectively to ensure patient comfort
• Collaborate with healthcare team members and community providers to coordinate care

Additional Information

• Schedule includes primarily weekdays with some weekend coverage
• 36 hours per week commitment
