OT Home Health - Travel/Contract

Location: San Ramon, California
Contract Length: 13 weeks


Job Details

  • Specialty: Therapy/Rehabilitation (Occupational Therapy)
  • Setting: Home Health
  • Shift: Day shift, 8 hours per day (8:00 AM - 4:30 PM), 5 days a week
  • Weekly Hours: 36 guaranteed hours (traveler must see 5-6 patients daily; guaranteed hours do not apply during first 2 weeks, holidays, or time off weeks)
  • Patient Ratio: 1:1, may visit up to 6 patients per day (27 productivity points standard)
  • Weekend Work: Optional to pick up patients to meet productivity goals
  • On-Call: Not required
  • Travel Radius: Occasional travel up to 50+ miles; system groups clients to minimize drive time
  • Transportation: Own transportation required
  • Scrubs: Any color allowed
  • Parking: Free
  • Mileage Reimbursement: IRS standard

Required Qualifications

  • Valid Occupational Therapist license in California
  • Minimum 1 year of Occupational Therapist experience, including experience within the last year
  • Experience in a Home Health setting
  • Current BLS (Basic Life Support) certification (AHA or ARC only)

Compliance & Documentation Requirements

  • Government-issued photo ID (Driver’s License/State ID)
  • Emergency Contact Form
  • I9 Attestation
  • Background Checks (7-year search including SS trace, county, nationwide, sex offender; updated annually)
  • EPLS/GSA/SAM and HHS/OIG checks (annual)
  • Additional certifications (ACLS, PALS) may be required per contract but BLS is mandatory
  • State License verification (primary source online verification)
  • Medication/Pharmacology Competency Exam as applicable
  • Personal or auto insurance (current throughout assignment)
  • Auto registration (current throughout assignment)
  • Physical exam within 1 year, cleared to work without restrictions
  • Immunizations and titers:
    • Tdap within 10 years or annual declination
    • Negative TB test or chest X-ray within 1 year
    • Measles, Mumps, Rubella: 2 vaccines or positive titers within required timeframes or boosters if negative
    • Hepatitis B: Titer, vaccine series, or declination updated annually
    • Varicella: 2 vaccines or positive titer, boosters if negative
  • Drug screen: 10-panel within 1 year
  • Flu vaccine or declination required year-round
  • COVID-19 vaccine documentation or declination form required (declinations accepted for religious/medical reasons)
  • References:
    • 2 professional references for travel/allied/per diem acute care
    • 1 professional reference for per diem non-acute care
  • Skills checklist and work history verification (1 year employment related to profession)

Additional Notes

  • Timely communication and documentation are required; documentation tools will be provided and must be returned
  • Locals are accepted for this position
