Job Overview

Role Summary

• Home health physical therapy role providing skilled home care visits.
• Typical caseload includes medically complex and post-surgical patients.
• Visits are self-scheduled based on home care office needs.

Required Qualifications

• Valid Physical Therapist license in Florida
• Physical Therapist (1 year of experience)
• Physical Therapist experience within the last year
• BLS - Basic Life Support issued by American Heart Association
• Experience in a Home Health setting
• Driver's License

Preferred Qualifications

• OASIS training

Key Responsibilities

• Assessment
• Cardiopulmonary/circulatory care
• Documentation
• Use of mobility and transfer equipment
• Orthopedic and neurological care
• Home exercise program instruction
• Medication-related support
• Patient and family education
• Safety and injury prevention
• Billing

Work Setting and Schedule Details

• 6–8 home visits per day
• Minimum of 6 points per day, with manager discretion to increase by 1 point
• 1–2 weekends per contract

Additional Notes

• Scrubs: any solid color
• Laptop, MiFi, and portable printer are used for the assignment
• Mandatory mask wearing applies when flu declination is on file or flu vaccination is not received
