Physical Therapist - Home Health (Travel/Contract)
Location: Mission Viejo, California
Contract Length: 13 weeks
Job Details
- Specialty: Home Health Physical Therapy
- Average Patients per Day: 5-7
- Visits: 6 visits daily or minimum of 30 points weekly
- Guaranteed Hours: 40 hours per week
- Shift: Day shift, 8:00 AM to 5:00 PM (5 days a week, 8 hours per day)
- Types of Cases: Geriatric
- Territory: Up to a 60-mile radius from the office/hub
- Dress Code: Scrubs (any color), no jeans
- Device Provided: iPad
- Cell Phone Use: Not allowed
- Mileage Reimbursed at IRS standard rate
- Locals accepted at travel rate
Required Qualifications
- Valid Physical Therapist license in California
- BLS (Basic Life Support) certification
- Experience with Homecare Homebase (HCHB) EMR
Compliance & Background Requirements
- Background checks must be current within 90 days of start and updated annually, including:
- SSN Trace
- County searches (last 7 years)
- FACIS Level 1
- Nationwide Sex Offender
- Motor Vehicle Records/Drivers Record
- Education Verification (highest degree related to profession)
- Work History Verification (7 years, with attempts to verify if needed)
- EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
- Fingerprinting requirements vary by state (e.g., Alaska, Connecticut, Washington) and must be completed as applicable
- State-specific criminal history and employability status checks required within 90 days of start for certain states (e.g., Texas, Washington)
Health & Safety Requirements
- Physical exam within the last 12 months (physician statement)
- Fit test for N95 mask (no specific model required)
- Negative TB test (PPD, Q Gold, or TSpot) within 1 year or chest X-ray within 5 years if positive PPD
- Annual TB questionnaire required for all candidates
- Immunizations or proof of immunity required for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B
- Varicella
- 9-panel drug screen within 12 months (instant acceptable)
- Seasonal flu vaccination or facility-specific declination form required during flu season
- COVID vaccination or facility-specific declination required
Documentation & Other Requirements
- Two professional references within 2 years of start (travel, allied, per diem acute care)
- Full 7 years of work history documented with no gaps (not required for extension flips)
- Annual skills checklist
- Proof of current auto insurance required (due to driving responsibilities)
- Signed orientation safety manual acknowledgment (updated annually)
- Completion of facility-specific acceptable use policy prior to start
- Facility-specific flu and COVID declination forms if applicable
Additional Notes
- No on-call responsibilities
- Compliance liaison manages background attestations and documentation submission
- Candidates must maintain timely and accurate documentation throughout the contract
