Schedule

Monday to Friday, 8:00 AM to 4:30 PM
Weekend rotation: 1 in 4 weekends
One weekend shift per month

Contract Length

13 weeks

Role Overview

Home care LPN role providing skilled nursing care to patients in the home
Approximately 6 patient visits per day
Works as part of a team with RNs, PTs, OTs, and HHAs

Required Qualifications

Valid LPN / LVN license in Massachusetts
Home Health experience within the last year
1 year of Home Health experience
BLS issued by American Heart Association or American Red Cross
Driver's License

Onboarding Requirements

Resume with the past 7 years of work experience and city/state for work and education
2 references within the past 4 years, including at least 1 supervisor or above in the skillset
Skills checklist completed within 1 year of submission
Driver’s license or state ID
MVR check
Criminal background checks and exclusion checks
Massachusetts CORI check
SSN trace
Education verification
National sex offender check
Fit test and respirator medical clearance, if required for the skill
TB screening
MMR documentation or immunity proof
Health statement
