Job Summary
Seeking a Registered Nurse with case management experience to work in an acute hospital setting. The role involves utilization management, care coordination, discharge planning, and revenue integrity within a 36-bed unit.

- Unit Type: Case Management
- Number of Beds: 36
- Number of Staff: 2
- Shift Types: Day shift with weekend rotation, night shift
- Patient Ratio: 18:1
- Equipment: EMR system (Midas)
Required Qualifications
- Valid RN license in New Hampshire
- Case Management experience (minimum 2 years), including experience within the last year
- Acute Hospital Case Management experience
- BLS certification
- Associate of Science in Nursing / Associate Degree in Nursing
- Critical care experience
- De-escalation skills in a healthcare setting
Work Hours
- Approximately 36 hours per week
