Shift Details
- Day shift, 4 days per week, 10 hours per day (7:00 AM – 5:30 PM)
- Rotating on-call required
Unit & Patient Population
- Unit specialties: GVHD (Graft-versus-host disease), BO (Bronchiolitis Obliterans), CTCL (Cutaneous T-cell lymphoma), Photopheresis
- Staffing ratio: 1:1
- Special equipment: CellEx
Required Experience & Skills
- Minimum 3 years of experience as a Registered Nurse
- Experience with Apheresis equipment or Dialysis
- Skilled in managing central lines and ports
- Plasma Exchange and Red Blood Cell Exchange experience required (pre-screening question)
Required Certifications & Licenses
- Indiana RN license (must be active and in hand before submission)
- BCLS (Basic Cardiac Life Support) certification required
- Other certifications may be required depending on cost center (e.g., ACLS, PALS, NRP, CHEMO, TNCC, EFM) – verify as applicable
Required Documentation & Compliance
- Valid Driver’s License or Passport (clear copy)
- Background checks including:
- 7-year work/lived history verification
- SSN Trace, County Criminal, Statewide, National Sex Offender, FACIS Level III
- Education verification
- EPLS/GSA/SAM and HHS/OIG checks within 30 days of start
- Transcript or diploma (official copy)
- Education verification by third party (may be pending at start)
- State license verification online within 30 days of start
- Physical exam within 12 months of start
- Fit mask test within 12 months (N95 or approved respirators)
- Negative 10-panel drug screen within 12 months
- Immunizations and titers:
- TB test (PPD, QuantiFERON-TB Gold, or T-SPOT) within 1 year
- Rubeola (Measles), Mumps, Rubella: positive titer or 2 vaccines required (no declinations accepted)
- Varicella: positive titer or 2 vaccines required (no declinations accepted)
- Hepatitis B series, titer, or signed declination required
- Tetanus/Diphtheria/Tdap preferred but not required
- Seasonal Flu vaccination or approved medical/religious exemption required during flu season
- Color blind exam (Ishihara or equivalent) with 100% minimum score
- Medication/Pharmacology competency exam and unit competency exam required
- References: minimum 2 from recent employment with supervisory titles, verified within 2 years
- Current employment application with full work history and explanations for gaps over 30 days (covering 7 years)
- Skills checklist completed within 12 months and annually thereafter
Additional Requirements
- Completion of facility-specific compliance and orientation documents prior to start (time sensitive)
- Rotating call availability
- Must complete any required facility-specific training modules as applicable
Requirements / Qualifications
- Valid RN license in Indiana
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Minimum 3 years of dialysis experience, including dialysis experience within the last year
- Experience with Plasma Exchange and Red Blood Cell Exchange
- Experience with Apheresis equipment
- Skilled in managing central lines and ports
- BLS certification issued by the American Heart Association
