Registered Nurse – Behavioral Health (Travel/Contract)

Location: Clearfield, Pennsylvania
Assignment Duration: 13 weeks
Shift: 12-hour shifts, either Day (7:00 AM – 7:00 PM) or Night (7:00 PM – 7:00 AM), typically 3 shifts per week (36 hours/week)

Job Description

• Work on a geriatric psychiatric unit with multiple dementia patients and/or patients requiring hands-on assistance with activities of daily living (ADL).
• Team-oriented environment; the RN will cover both RN and LPN shifts during orientation of new staff.
• Patient ratio: maximum 10 patients with 3 staff members.
• Charting system: Cerner.
• Some weekend shifts required.
• No floating required.

Required Qualifications

• Valid Registered Nurse (RN) license in Pennsylvania.
• Minimum of 1 year of Behavioral Health experience within the last year.
• BLS (Basic Life Support) certification issued by the American Heart Association or American Red Cross.
• ACLS (Advanced Cardiac Life Support) certification issued by the American Heart Association or American Red Cross.
• NIH Stroke Scale (NIHSS) certification.
• Certification in Management of Assaultive Behavior (MAB), Crisis Prevention Institute (CPI), or Comprehensive Crisis Management (CCM).
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
• Valid Driver’s License.

Required Background and Documentation

• Government-issued photo ID (Driver’s License or State ID).
• 7-year criminal background check including SS trace, county searches for all locations lived/worked/schooled, nationwide, sex offender, and Pennsylvania statewide (Patch Certificate).
• EPLS/GSA/SAM and HHS/OIG checks.
• Education verification (transcripts, diplomas, or third-party verifications).
• Pennsylvania Child Abuse Search (Childline) within 1 year of start.
• Pennsylvania Department of Human Services FBI fingerprinting within 12 months of start (must be completed in PA; no mailing of fingerprint packets).
• Two professional references within the last 3 years.
• Annual skills checklist.

Health and Safety Requirements

• Physical exam within 1 year of start.
• Negative TB test (PPD, Q Gold, or TSpot) within 6 months or chest X-ray within 5 years. Positive TB requires verbal confirmation and annual questionnaire.
• Immunizations or positive titers for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella (Chickenpox)
• Tdap vaccination within the past 10 years (declination accepted with form).
• Seasonal flu vaccination or facility-specific declination (mask required if declined during flu season Nov-April).
• COVID vaccination or facility-specific declination.
• Negative 10-panel drug screen within 30 days of start.
• Alcohol breathalyzer test within 30 days of start (not required at this location).
• Photo for badge (passport style, no filters).
• Completion of facility-specific orientation packet, medical surveillance checklist, and bloodborne pathogen quiz prior to start.
• Completion of vaccine declination forms if applicable (COVID, flu, MMR, Hep B, Tdap).
