Employment Type: Long-Term Assignment (LTA)
Contract Length: 12 weeks

Job Description

• Provide physical therapy primarily in patients' residences.
• Assess patients' current conditions and develop individualized treatment plans.
• Focus on pain relief and improving patient mobility.
• Create patient-specific fitness programs based on trauma root cause.

Required Qualifications

• Valid Physical Therapist license in California
• Bachelor of Science degree in Physical Therapy
• Current Basic Life Support (BLS) certification issued by the American Heart Association

Required Onboarding Credentials and Health Requirements

• Professional state license verification (including acceptance of out-of-state licenses if permitted).
• BLS certification.
• COVID-19 vaccine.
• MMR, Hepatitis B, Influenza vaccine, Varicella, Tdap.
• Tuberculosis (TB) test and annual TB questionnaire.
• Physical/health statement.
• Criminal background check (covering county, state, and national levels within 6 months, with 7-year history).
• Employment verification and education verification.
• Resume and cover page (using specified template).
• Two professional references.
• Confirmation of assignment.
• I9 documentation and attestation.
• Motor vehicle report and attestation.
• Social security trace (within 6 months, showing actual addresses).
• Drug screen.
• Skills checklist and core competencies.
• Unit competency exam.
• Webchecks.

Shift

• 8:00 AM to 4:30 PM

Additional Notes

• Must have dependable transportation for travel to patient homes.
