Required Qualifications

• Valid RN license in Michigan
• Med Surg / Telemetry (1 year of experience)
• Med Surg / Telemetry experience within the last year
• BLS issued by American Heart Association
• Associate of Science in Nursing / Associate Degree in Nursing

Onboarding Requirements

• COVID-19 vaccine documentation, declination, or exemption with agency letterhead is required.
• 10-panel drug screen completed within 30 days of start is required.
• MMR documentation is required.
• Fit test must be completed on site within one year of start.
• Hepatitis B series or declination is required.
• 2-step PPD or IGRA (Quantiferon/Tspot) within one year of start is required.
• Varicella documentation is required.
• Tdap documentation is required.
• Physical/health statement from an exam within the last year is required.
• Confidentiality and information security agreement must be signed.
• Conflict of interest disclosure form must be signed.
• Criminal background check, SSN verification, education verification, EPLS, GSA/OIG, integrity and audit services form, NSO, system access form, code of conduct form, I-9, specialty-specific competency testing, color vision, respirator evaluation, and Joint Commission testing are required as part of onboarding.

Additional Notes

• American Heart Association BLS is required for all direct patient care personnel.
• License/certificate verification must be dated within 30 days of start.
• Specialty exam and CORE exam must be completed within one year.
