Physical Therapist - Contract Position

Location: San Ramon, California (Assigned territory includes San Jose, Santa Clara, Sunnyvale, Cupertino, Milpitas, Mountain View, Campbell, Los Altos, Saratoga, Los Gatos)

Job Description

Seeking a qualified Physical Therapist to provide rehabilitation therapy services. The role requires travel within the assigned county territory and may include additional travel outside the area. Weekend coverage may also be required.

• Must be willing to travel outside the assigned area or work weekends to maintain weekly visit expectations.
• May be required to see COVID-positive and COVID PUI patients.
• Productivity expectations include completing a minimum number of patient visits per week (points system based on visit types).
• Hours approved on a per-visit basis, covering drive/travel time, visit time, documentation, and communication with patients and doctors.

Required Qualifications

• Valid Physical Therapist license in California
• Minimum of 1 year of Physical Therapist experience, including experience within the last year
• Advanced Cardiovascular Life Support (ACLS) certification
• Basic Life Support (BLS) certification
• Pediatric Advanced Life Support (PALS) certification
• Neonatal Resuscitation Program (NRP) certification
• Valid Driver’s License and reliable transportation
• Up-to-date immunizations and screenings including:

  • TB (annual)
  • MMR
  • Varicella
  • Hepatitis B
  • Tdap (within 10 years)
  • Annual Flu vaccine
• Drug screen and physical exam clearance

Additional Information

• Agencies must ensure contract dates and requested time off are fully disclosed and match confirmation details to avoid removal from the vendor set.
• Time off requests submitted after acceptance will not be approved.
