Job Opportunity: RN - Case Management (Travel)

Location: San Leandro, California
Employment Type: Travel

Job Description

Coordinate management of care to ensure optimal resource utilization, service delivery, and compliance with external review agencies.
Provide comprehensive assessment, care planning, plan implementation, and evaluation of individual patient needs.
Enhance quality of patient management and satisfaction, promoting continuity of care and cost effectiveness.
Integrate case management, utilization review, and discharge planning functions.
Serve as a patient advocate addressing holistic patient needs through resource utilization and discharge planning.
Collaborate strongly with interdisciplinary teams (IDT).

Schedule

Monday to Friday, 8:00 AM – 4:30 PM

Contract Length

13 weeks

Required Qualifications

License: California RN license required.
Certifications: BLS certification through the American Heart Association required.
Experience: Minimum 2 years recent RN Case Management experience required.
Experience must include LTAC or STACH settings, MCG guidelines, and discharge planning across acute care settings.
Strong interdisciplinary team collaboration skills required.
